PREREQUISITES:
The established criteria and procedure used for admission to the class bring a copy of
1.- Photo identification.
2.- Social security card.
3.- Require student to have C.P.R prior to the clinical component of the course and provide the Copy of the C.P.R.
4.- Copy of the certification of approved radiation safety program
5.- Work experience or completion of approved RDA program.
6.- Must satisfactory demonstrate to the instructor clinical competency in infection control Requiems prior to clinical instruction in pit and fissure sealant
7.- Copy of certification of completion of approved corona polishing program.
ADMISSIONS PROCEDURE:
1. Meet the criteria.
2. Fill the admission application
3. Bring California ID and social security card, if student is under the age of 18 a parent or legal guardian must sign the application. Identification of legal guardian will be need as well.
5. No application fees. A non-refundable fee of $100 will be applied for any cancelled classes.
